If you can’t make an event and scrolling isn’t your thing, feel free to download the checklists and cheat sheets. Our checklists are based on the Jean Hailes list of health checks. They are amazing resources that we strongly recommend following. Please note that due to changes in services provided in our area, these may have incorrect information after some time. We’ll do our best to keep them current but as a volunteer service, can’t guarantee a quick update.
Checklists
This statement from the Jean Hailes Health Check page sums up why we’ve created the Check Me Out project. “How do you know if your blood pressure is too high or too low, if your cholesterol is within a healthy range, or whether you should have a blood-sugar test, a cervical screening test or immunisation review? Knowing the numbers of key health measures can help you to know if you need to make some changes for the benefit of your current and future health”.
The checklists below have been created using the Jean Hailes health checks. If you can’t make one of our events you are welcome to download the checklist and fill it out yourself in your own time at home.
20 - 40 Years Checklist
“Regular health checks and screenings, in combination with a healthy diet and regular physical activity, can assist in the prevention of disease or illness. Recommendations are made for how often you should have a blood pressure, Cervical Screening Test, blood sugar, breast, skin and immunisation review” . Jean Hailes

40-60 Years Checklist
Did you know that the government offers a '45-49-year-old health check' – a once-only check with your doctor for those at risk of developing a chronic disease.
The complete range of medical checks and screening procedures required throughout life will vary for every person, depending on individual risks, medical background and family history. These checklists are a great way to start a conversation with your doctor about the tests that you may need.
Over 60 Years Checklist
Did you know that the government offers a '75+ year-old health check' – a free annual health assessment with your doctor for people aged 75 years and over.
